Over the last 2 weeks, how often were you bothered by each problem?
| Points | Frequency |
|---|---|
| 0 | Not at all |
| 1 | Several days |
| 2 | More than half the days |
| 3 | Nearly every day |
| Total score | Band | Educational next step |
|---|---|---|
| 0–4 | Minimal | Monitor; reinforce sleep, activity, and stress hygiene |
| 5–9 | Mild | Self-care + consider primary care if persistent several weeks |
| 10–14 | Moderate | Formal evaluation; CBT and/or medication often discussed |
| 15–21 | Severe | Prompt behavioral-health follow-up; assess safety |
Clinicians may use ≥10 as a common threshold to discuss treatment; ≥15 suggests high burden — always individualize.
Screening tools quantify symptom frequency to support conversations and track treatment response. They do not replace clinical diagnosis, which considers duration, impairment, differential diagnosis (panic disorder, PTSD, OCD, adjustment disorder), and medical workup when indicated.
Crisis care is separate: thoughts of self-harm, suicide plans, or inability to stay safe require immediate help (U.S. 988 Suicide & Crisis Lifeline). High scores plus safety concerns are emergencies, not “schedule therapy in a month.”
Crisis: If you may harm yourself or are in immediate danger, call emergency services or 988 (U.S.). This calculator is not emergency triage.
